Which Type of Creatine Is Best?

Walk into a supplement store and you may see numerous varieties:

  • Creatine monohydrate

  • Creatine hydrochloride

  • Buffered creatine

  • Creatine nitrate

  • Micronized creatine

  • Creatine blends

For most people, the simplest answer is also the best:

Creatine Monohydrate

Creatine monohydrate is the most widely studied form and is generally the form we recommend when supplementation is appropriate. (Office of Dietary Supplements)

More expensive forms have not consistently demonstrated meaningful advantages over standard creatine monohydrate.

How Much Creatine Should You Take?

A common maintenance dose is:

3–5 grams of creatine monohydrate daily

Another strategy involves a loading phase:

Approximately 20 grams per day, divided into four doses, for 5–7 days, followed by 3–5 grams daily. This protocol has commonly been used in research. (Office of Dietary Supplements)

However, loading is not required.

Taking a smaller daily dose will gradually increase muscle creatine stores without a loading phase.

For most patients interested in general health and fitness, simplicity and consistency are more important than complicated dosing schedules.

When Should You Take Creatine?

Creatine does not need to be taken immediately before a workout to work.

The most important factor is consistent daily use.

You can take it:

  • In the morning

  • With a meal

  • Before exercise

  • After exercise

  • Mixed into a shake or beverage

Choose a routine that makes it easy to remember.

Does Creatine Damage the Kidneys?

This is probably the most common concern surrounding creatine.

Current evidence does not show that recommended creatine supplementation damages kidney function in healthy individuals. Recent systematic reviews found that creatine can cause a modest rise in serum creatinine without a corresponding significant reduction in measured or estimated kidney filtration. (PubMed)

There is an important reason for this.

Creatine can increase serum creatinine, a laboratory marker commonly used to estimate kidney function.

Therefore, someone taking creatine may have a slightly higher creatinine blood test without actually having kidney damage.

Tell your healthcare provider that you take creatine when having kidney function testing.

People with known kidney disease or other significant medical conditions should discuss supplementation with their healthcare provider before starting it.

Is Creatine Safe?

Creatine is generally considered well tolerated in healthy adults when used appropriately. NIH information notes that it appears safe for longer-term use over several years, and a large 2025 analysis of hundreds of clinical trials also evaluated its safety profile. (Office of Dietary Supplements)

Possible effects include:

  • Water-weight gain

  • Bloating

  • Gastrointestinal discomfort

  • Occasional cramping or stiffness

Taking smaller doses rather than large amounts at once may reduce gastrointestinal symptoms.

Does Creatine Cause Hair Loss?

This concern frequently circulates online.

At present, there is not strong clinical evidence demonstrating that creatine supplementation causes hair loss.

Much of the concern originated from limited research involving changes in a hormone associated with androgen activity rather than studies actually demonstrating increased hair loss.

More research would be helpful, but current evidence does not justify telling patients that creatine is a proven cause of baldness.

Does Creatine Cause Dehydration?

Creatine draws water into muscle cells, which sometimes leads people to assume it causes dehydration.

Current evidence does not support routine creatine use causing dehydration in healthy individuals when used appropriately.

Adequate hydration remains important, particularly during exercise, hot weather, or prolonged physical activity.

Who Should Talk With a Healthcare Provider Before Taking Creatine?

Medical guidance is particularly important for people who:

  • Have kidney disease

  • Have significant liver disease

  • Are pregnant or breastfeeding

  • Take medications that may affect kidney function

  • Have complex chronic medical conditions

  • Are adolescents considering supplementation

  • Have abnormal kidney laboratory results

Supplements can also vary significantly in quality, so choosing reputable products with independent third-party testing is important.

Creatine Is Not a Substitute for the Basics

No supplement can replace the fundamentals of healthy aging and physical performance.

For optimal muscle and metabolic health, focus on:

  • Resistance training

  • Adequate protein

  • Regular cardiovascular activity

  • Whole-food nutrition

  • Healthy body composition

  • Adequate hydration

  • Quality sleep
